What Are Multiple Follicular Units Grafts?
Linear Multiple Follicular Unit (MFU) grafts represent an advancement in hair transplantation. Follicular Unit (FU) grafts contain an average of 2.2 hairs per graft. MFU grafts contain 2 to 3 follicular units per graft, yielding 6 to 8 hairs per graft. This technique allows for greater hair density and enhanced coverage, resulting in natural-looking, fuller hair.
In scalp hair transplantation, Dr. Arthur Zacco commonly uses FU grafts along the hairline and temples to create a natural, feathered look, while MFU grafts are placed behind the hairline to increase density and fullness.

Traditional Plugs
Historically, traditional plugs were about 4 mm wide and looked unnaturally and ‘doll-like.’ These large round plugs averaged 10 follicular units (20 hairs). The natural-looking linear MFU graft averages 2.5 follicular units each (about six hairs). Since the newer linear MFU grafts are thin and smaller, they deliver density while appearing natural, the best of both worlds.
Dr. Zacco’s innovative approach with Linear MFUs has revolutionized the process, maintaining the thinness of FU grafts while incorporating multiple groupings of follicles. This allows for grafts to be placed closer together, creating a more natural-looking density across larger areas. The result is a dense and natural appearance.
FUT (Strip Method) vs. FUE
The FUT (Strip Method) offers significant advantages, particularly in its ability to transplant a larger number of follicles in each session. FUT can move 2-3 times more follicles compared to FUE. By producing both FU and MFU grafts, FUT/Strip hair transplants allow for greater hair density in the recipient areas.
With FUE, there is a higher follicular transection rate (30-50%) because follicles are harvested blindly, as they are beneath the skin’s surface. In contrast, FUT/Strip procedures have a much lower follicular transection rate (approximately 3%), as the follicles are carefully visualized under a microscope before being grafted, ensuring better preservation and less risk of damage.
FUE is ideal for small areas and offers the benefit of minimal scarring, with tiny scars that can be hidden with short haircuts (#1-2 clipper length). The recovery time for FUE at the donor site is about 3-4 days, whereas FUT requires about 8-10 days. However, recovery time for the recipient area is the same for both methods—approximately 8-10 days. However, when considering a cosmetic procedure whose results will last a lifetime, the recovery time difference of a few days becomes less important.
With FUT, the risk of depleting the donor hair supply is minimized, ensuring that there is enough hair available for both current and future balding areas. In contrast, FUE can exhaust the donor follicles before all balding areas are treated, making FUT/Strip a better option for individuals with extensive hair loss and anticipated future hair loss.
Even though FUE scars can be concealed by shorter hair, each hair transplantation results in about 2-3x more scar tissue compared to the FUT (Strip). Since no hairs grow out of scar tissue, every subsequent hair transplantation produces new scars. With every subsequent FUT (strip) hair transplantation, the old scar is removed, which results in one remaining scar. This produces a limiting factor with FUE procedures. Using the FUE technique, many times, the donor area is used up before the recipient area is completed.
